巨噬细胞相关蛋白与TIM-3在IgG4相关性唾液腺炎中的表达及临床意义

The expression and clinicopathological significance of macrophage-associated protein and TIM-3 in IgG4-related sialadenitis

摘要: 目的:探讨巨噬细胞相关蛋白(CD68、CD80、CD163)及TIM-3蛋白在IgG4相关性唾液腺炎(immunoglobulin G4-related sialadenitis,IgG4-RS)中的表达及与临床病理特征的相关性。方法:选取33例IgG4-RS患者、8例舍格伦综合征(Sjögren syndrome,SS)患者及8例健康对照组的唾液腺组织标本,通过HE染色、Masson染色及免疫组织化学染色技术,检测CD68、CD80、CD163、TIM-3蛋白的表达水平;分析上述蛋白表达与患者年龄、累及器官数、组织 IgG4/IgG 阳性浆细胞比值及纤维化面积的相关性。结果:IgG4-RS患者唾液腺组织中CD68、CD163、TIM-3的表达水平显著高于SS组(P<0.05);CD68的表达与患者年龄及累及器官数目相关(P<0.05),CD80的表达与累及器官数目相关(P<0.05),CD163的表达与IgG4/IgG 阳性浆细胞比值及年龄相关(P<0.05),TIM-3的表达与年龄相关(P<0.05)。Spearman相关性分析显示,IgG4-RS患者腺体组织中CD68、CD163、TIM-3的表达量与纤维化相对面积呈正相关(P<0.05)。结论:IgG4-RS唾液腺组织中CD68、CD163与TIM-3蛋白呈高表达状态,且其表达水平与患者年龄、累及器官数目及组织纤维化程度密切相关,为揭示IgG4-RS的免疫发病机制、优化临床诊断与治疗方案提供了新的理论依据。

Abstract: PURPOSE: To investigate the expression characteristics of macrophage-associated proteins (CD68, CD80, CD163) and TIM-3 protein in IgG4-related sialadenitis (IgG4-RS) and their correlation with clinicopathological parameters of patients. METHODS: Salivary gland tissue samples from 33 patients with IgG4-RS, 8 patients with Sjögren syndrome (SS) and 8 healthy controls were selected. The expression levels of CD68, CD80, CD163 and TIM-3 proteins in the three groups were detected by HE staining, Masson staining and immunohistochemical staining. The correlations between the expression of these proteins and patients' age, number of involved organs, ratio of IgG4/IgG positive plasma cells, and fibrosis area were analyzed. RESULTS: The expression levels of CD68, CD163 and TIM-3 in the salivary gland tissues of patients with IgG4-RS were significantly higher than those in the SS group (P<0.05). CD68 expression was correlated with patients' age and number of involved organs (P<0.05), CD80 expression was correlated with the number of involved organs (P<0.05), CD163 expression was correlated with the ratio of IgG4/IgG positive plasma cells and age (P<0.05), and TIM-3 expression was correlated with age (P<0.05). Spearman correlation analysis showed that the expression levels of CD68, CD163 and TIM-3 in the glandular tissues of IgG4-RS patients were positively correlated with the relative area of fibrosis (P<0.05). CONCLUSIONS: CD68, CD163 and TIM-3 proteins are highly expressed in the salivary gland tissues of IgG4-RS, and their expression levels are closely related to patients' age, number of involved organs, and degree of tissue fibrosis. This study provides a new theoretical basis for revealing the immune pathogenesis of IgG4-RS and optimizing clinical diagnosis and treatment strategies.
