223例头颈部黏液表皮样癌预后分析

Prognostic analysis of 223 cases of mucoepidermoid carcinoma in the head and neck

摘要: 目的:探讨头颈部黏液表皮样癌(mucoepidermoid carcinoma,MEC)的临床特点,分析其发病、转移、复发以及预后的相关因素,为临床诊疗与预后评估提供参考。方法:收集2012—2024年在山东第一医科大学附属省立医院就诊的223例头颈部MEC患者的临床资料,通过门诊以及电话随访获取完整预后数据。采用GraphPad Prism 10软件的 Kaplan-Meier 法计算生存率,采用SPSS 27.0软件的Cox比例风险回归模型进行单因素分析,对有统计学意义的因素进行多因素分析。结果:223例MEC患者的总体生存率为78.92%,复发率为6.28%,转移率为11.21%。其中,复发患者生存率为64.29%,转移患者生存率为76.00%。Cox回归分析显示,年龄、N分期、组织分化程度是患者预后生存的影响因素;生存曲线分析显示,低分化患者生存率显著低于中高分化患者(P<0.001)。结论:头颈部MEC患者的预后影响因素具有分层特征,总体生存率与年龄、N分期及组织分化程度密切相关,复发风险主要受N分期、组织分化程度影响,发生转移则与神经侵犯及组织分化程度显著相关。

Abstract: PURPOSE: To explore the clinical characteristics of mucoepidermoid carcinoma (MEC) in the head and neck region, analyze the related factors of its onset, metastasis, recurrence and prognosis, and provide references for clinical diagnosis, treatment and prognosis evaluation. METHODS: The clinical data of 223 patients with head and neck MEC who were treated at Shandong Provincial Hospital Affiliated to Shandong First Medical University from 2012 to 2024 were collected, and complete prognostic data were obtained through outpatient and telephone follow-up. The Kaplan-Meier method in GraphPad Prism 10 software was used to calculate the survival rate, and Cox proportional hazards regression model in SPSS 27.0 software was adopted for univariate analysis. Further multivariate analysis was performed on the factors with statistical significance. RESULTS: The overall survival rate of the 223 MEC patients was 78.92%, the recurrence rate was 6.28%, and the metastasis rate was 11.21%. Among them, the survival rate of recurrent patients was 64.29%, and that of metastatic patients was 76.00%. Cox regression analysis showed that age, N stage, and histological differentiation were factors affecting patient prognosis. Survival curve analysis revealed that patients with low differentiation had significantly lower survival rates compared to those with moderate or high differentiation (P<0.001). CONCLUSIONS: The prognostic factors of patients with head and neck MEC have hierarchical characteristics. The overall survival rate is closely related to age, N stage, and the degree of tissue differentiation; the risk of recurrence is mainly affected by N stage and the degree of tissue differentiation; the occurrence of metastasis is significantly correlated with perineural invasion and the degree of tissue differentiation.
