精细化护理模式在双侧唇裂术后继发鼻唇畸形矫治患儿手术中的应用效果评价

The effect of refined nursing model in children with secondary nasolabial deformity correction after bilateral cleft lip surgery

摘要: 目的:探讨精细化护理模式对双侧唇裂术后鼻唇畸形矫治患儿术后康复及美学效果的影响。方法:选取2023年1月—2025年1月在广西医科大学第一附属医院接受双侧唇裂术后鼻唇畸形矫治手术的患儿86例,随机分为对照组与试验组,每组各43例。对照组采用常规护理,试验组在常规护理基础上联合精细化护理。统计两组术后并发症发生率、伤口愈合时间及住院时间;采用数字疼痛评分量表(Numerical Rating Scale,NRS)、Face-Q量表、儿童抑郁障碍自评量表(Self-Rating Scale of Depressive Disorder in Children,DSRSC)、儿童焦虑性情绪障碍筛查表(Screen for Child Anxiety Related Emotional Disorders,SCARED)及本院自制护理满意度问卷,评估患儿疼痛、面部美观度、心理状态及家属护理满意度。结果:试验组术后并发症总发生率(4.65%)显著低于对照组(23.26%)(P=0.013)。随着时间延长,两组患儿NRS评分逐渐降低(P<0.05)。术后12、24、48 h,试验组NRS评分显著低于对照组(P<0.05)。与对照组相比,试验组术后创口愈合时间、住院时间更短(P<0.05)。术后3个月,试验组患儿Face-Q量表中口腔恢复状况、鼻部形态和对称性、唇部形态和对称性、瘢痕质量和颜色匹配、瘢痕恢复情况评分均显著高于对照组(P<0.05)。术后3个月,两组DSRSC、SCARED评分均显著下降(P<0.05),且试验组评分显著低于对照组(P<0.05)。试验组面部美学、服务态度、环境、专业技能、护理效果等维度满意度评分显著高于对照组(P<0.05)。结论:精细化护理模式可有效降低双侧唇裂术后鼻唇畸形矫治患儿术后并发症发生率、术后疼痛程度,缩短康复时间,提高面部美观度和患儿家属护理满意度,改善患儿心理状态。

Abstract: PURPOSE: To explore the effect of refined nursing model on postoperative rehabilitation and aesthetic outcomes in children undergoing correction of nasolabial deformity after bilateral cleft lip surgery. METHODS: A total of 86 children who received surgical correction for nasolabial deformity following bilateral cleft lip repair in the First Affiliated Hospital of Guangxi Medical University from January 2023 to January 2025 were enrolled and randomly divided into the control group and the experimental group, with 43 cases in each group. The control group was given routine nursing, while the experimental group received refined nursing combined with routine nursing. The incidence of postoperative complications, wound healing time and hospital stay were compared between the two groups. The Numerical Rating Scale (NRS), Face-Q Scale, Self-Rating Scale of Depressive Disorder in Children (DSRSC), Screen for Child Anxiety Related Emotional Disorders (SCARED), and a self-designed nursing satisfaction questionnaire of the hospital were adopted to evaluate children's pain intensity, facial aesthetics, psychological status, and nursing satisfaction of their families. RESULTS: The total incidence of postoperative complications in the experimental group was 4.65%, which was significantly lower than 23.26% in the control group (P=0.013). The NRS scores of both groups decreased gradually over time (P<0.05); at 12, 24 and 48 hours after surgery, the NRS scores in the experimental group were lower than those in the control group (P<0.05). Compared with the control group, the experimental group had shorter postoperative wound healing time and hospital stay (P<0.05). At 3 months postoperatively, the scores of oral recovery, nasal morphology and symmetry, labial morphology and symmetry, scar quality and color matching, as well as scar recovery in the Face-Q Scale of the experimental group were significantly higher than those of the control group (P<0.05). At 3 months after surgery, the DSRSC and SCARED scores of both groups declined (P<0.05), and the scores in the experimental group were markedly lower (P<0.05). The satisfaction scores of the experimental group in dimensions including facial aesthetic improvement, service attitude, hospitalization environment, professional skills and nursing efficacy were significantly higher than those of the control group (P<0.05). CONCLUSIONS: The refined nursing mode can effectively reduce the incidence of postoperative complications and postoperative pain, shorten the rehabilitation period, improve facial aesthetics and family nursing satisfaction, and optimize the psychological state of children undergoing nasolabial deformity correction after bilateral cleft lip surgery.
