头颈部多间隙感染严重并发症危险因素的回顾分析

A retrospective study of risk factors for life-threatening complications of multi-space infections of head and neck

摘要: 目的 通过大样本数据分析头颈部多间隙感染严重并发症的危险因素。方法 回顾分析2006年2月—2014年7月549例头颈部多间隙感染患者的临床资料。将患者分为糖尿病组和非糖尿病组,统计分析头颈部多间隙感染严重并发症的高危因素。采用SPSS 19.0 软件包对数据进行统计学分析。结果 60~69岁患者头颈部多间隙感染的严重并发症发病率最高,为18.8%;与20~59岁年龄组差异有统计学意义(P=0.027)。多因素方差分析发现,糖尿病组严重并发症发生率是非糖尿病组的2.404倍(P=0.008)。结论 高龄和糖尿病是头颈部多间隙感染严重并发症的高危因素,建议在头颈部多间隙感染的诊治过程中,高度关注高龄(60岁以上)和糖尿病患者。

Abstract: PURPOSE: To assess the risk factors (age, gender, and underlying systemic diseases) of life-threatening complications of multi-space infections (LCMI) in the head and neck. METHODS: This was a retrospective cohort study that enrolled a sample of patients with multi-space infections who were referred from February 2006 through July 2014. The patients were classified into diabetic and nondiabetic group. Statistical analysis was performed using SPSS 19.0 software package. RESULTS: The highest incidence of LCMI was in 60-69 yr group (18.8%), which was significantly higher than 20-59 yr group (P=0.027). The correlation was further confirmed by multivariable analysis (odds ratio, 1.148; P=0.041). The odds of susceptibility for LCMI development in diabetic patients was 2.404 times as that of nondiabetic patients (odds ratio, 2.404; P=0.008). CONCLUSIONS: Aging and diabetes mellitus increase the risk of development of life-threatening complications in patients with multi-space infections of head and neck. To increase the chance of patient survival, we strongly recommended clinicians pay more attention to patients with multi-space infections, especially those over the age of 60 and/or diabetic patients.
